Permission and Liability Waiver

I hereby grant permission for my child to participate in the activities organized by Philadelphia Baptist Church. I acknowledge that participation in the event involves inherent risks. I understand that Philadelphia Baptist Church and its staff will take reasonable precautions to ensure the safety of all participants, but cannot eliminate all risks.

I, on behalf of myself, my child, and our heirs, agree to release, waive, and hold harmless Philadelphia Baptist Church, its pastors, staff, volunteers, and affiliates from any liability, claims, or demands arising from injury, loss, or damage to person or property during the event, except in cases of gross negligence or willful misconduct.

In the event of a medical emergency, I authorize Philadelphia Baptist Church staff to seek and consent to medical treatment for my child, including but not limited to first aid, emergency transport, or hospital care, if I cannot be reached. I understand that every effort will be made to contact me or the designated emergency contact promptly.

I grant permission for Philadelphia Baptist Church to use photographs or videos of my child taken during the event for church-related promotional or informational purposes, such as on the church website, social media, or printed materials.

I agree that my child will abide by all event rules and guidelines set forth by Philadelphia Baptist Church, including behavior expectations, safety protocols, and any restrictions (e.g., respectful conduct, no personal electronics). I understand that failure to follow these rules may result in my child’s dismissal from the event.

I have read, understood, and agree to the above terms and conditions. I voluntarily sign this waiver and consent form.
